Interprovincial Child Welfare Record Checks
The Children’s Aid Society of Ottawa (“the Society”) completes Interprovincial Child Welfare Records checks for Child Protection Agencies outside of Ontario and Indigenous Child Welfare Agencies within Ontario for the purposes of screening prospective adoption/foster/kin families/individuals.
Consents from all individuals over the age of 16 that you are seeking information about is required. You may use your own consent form, as long as it is detailed and specific to the Society releasing its information to you, or you may use the Society’s consent form here. For children under the age of 16, parental consent is required for the release of their information. This is to be clearly noted on the consent form.
For Interprovincial Request for Services relating to an open protection matter, you may call the Society’s Telephone Intake Unit (613-747-7800) for verbal disclosure of the family in question’s involvement. If you still wish to receive the records pertaining to the family involved in your open investigation, please complete the Interprovincial Request for Services form here. Please clearly indicate an open investigation is occurring on the form. Consent is not required for an open investigation for a protection matter.

Outside Placement Resource (OPR) Child Welfare Record Checks
If you are applying for a position with a privately owned and operated youth group home facility in Ottawa, please complete the Request for Child Welfare Records Check form that your potential employer provided to you. You will need to send this form along with one piece of valid government issued photo identification.
Your request will only be processed once all required information has been received in a legible format. If your Request for Child Welfare Check form is illegible or if sections are incomplete delays will occur or your request will not be processed.
By submitting a Child Welfare Record Check Consent Form, you are consenting to a search of the Child Protection Information Network (“CPIN”) and the Society’s internal records system.
Once your request has been completed by the Society, the results will be sent directly to the requestor.

Private Adoption Child Welfare Record Checks
The Society completes Child Welfare Record Checks for Private Adoption Agencies for the purposes of screening prospective adoptive parents. Please submit your request in writing and provide the following detailed and accurate information regarding the individuals (to assist in completing a thorough and accurate records search):
- Full names (current and past)
- Dates of birth
- If possible – names and dates of birth of parents/siblings/former spouses, children
- Previous addresses
- Current phone numbers and address
