Adoption Questionnaire

If you are considering adoption through The Children’s Aid Society of Ottawa, please review this information. We ask that you complete the questionnaire below to be contacted by an adoption worker who will help you to determine your readiness to take on the rewards and challenges of this life-long commitment. 

To be eligible to adopt in Canada, you must either be a Canadian Citizen or a permanent resident. In addition, The Children’s Aid Society of Ottawa strives to maintain children and youth within our jurisdiction. If you do not reside within the Ottawa jurisdiction, we ask that you contact your local Children’s Aid Society to learn more about the adoption needs in your area.  

To obtain more information about adoption in general and adoption though the child welfare system, the first step is to speak to a worker at the Adoption Council of Ontario’s (ACO) Centralized Intake team (1-877-ADOPT-20, 9am to 5pm, Monday to Friday), as well as to review the information on their website.  Additional webinars and information can also be found on the main ACO website.

After contacting the ACO, if your interests meet the needs as explained, we ask that you please provide us with the information outlined in the Adoption Questionnaire below. The questions are designed to inform you of the next steps, based on your individual/family situation. By completing this information, you are providing your consent for The Children’s Aid Society of Ottawa to document all of this information in the provincial child welfare database, the Child Protection Information System (CPIN).
